Oil and Gas Permits

As per the Updated Oil and Gas Policy, permits (MM-109) are require for any work within the Division of Highways's right of way.  Please fill out an Oil and Gas Data Information Sheet to begin the application process.  Once filled out, this sheet can be printed (8 1/2" x 14" paper) and mailed or emailed to the to the appropriate District Oil and Gas Coordinator.  You can find the appropriate person listed on the Local Contacts under each District from the Districts page of this website.

*Please note that receiving an approved MM-109 permit does not releave you from getting the appropriate over-weight/height/length hauling permit. Please see information contained on permit. Please see information contained on permit. Please see information contained on Order Permits page.

Please be aware that the WVDOH has additional Oil and Gas Pipeline Requirements beyond what is in the Utilities Manual.

Applicant will be required to have an appropriate bond and Road Maintenance Agreement in place before receiving an approved permit.
