We are pleased to announce that we have adapted a new solution that allows you to submit payments online, via a secure form, without the hassle of writing and mailing a check. The form is a secure form through the WV State Treasury’s Office that requires an invoice number. Please see the below form for select companies’ invoice numbers. If you are an individual, please use the prefix TR- and add your last name as your invoice number. (Example: TR-Doe) Please contact

We are looking forward to the transition and hope you enjoy the simplified process as well. You may still send in checks as you have been if you prefer that method.

Application Fee: $100.00 (for non-DOH Employees only)
NOTE: The $100.00 application review fee will be waived for all college graduates that apply within one year of graduation.
Biennial Renewal Maintenance Fee: $50.00 due by December 31st each odd year (for non-DOH employees only)
Late Fees:
- $10.00 after 1st month
- $20.00 after 2nd month
- $30.00 after 3rd month
After March 31st, certification will be revoked.
